The Centre for Cardiovascular Telemedicine provides remote patient management in heart failure patients. Patients will be trained by nurses in performing daily measurements of ECG, blood pressure and weight. The data will be transmitted into the electronic health records and interpreted by medical professionals in the telemedical centre. In case of deterioration, different activities will started: contacting the patient, changing medication, requesting an outpatient visit or calling emergency services. This care concept should help to reduce mortality and morbidity for specific groups of heart failure patients.
